1. Accomplished Application Form (2 sets of Form 1 with passport size photo)
2. Transcript of Records with graduation date and GWA/GPA
   Baccalaureate degree in Sciences (Health Sciences is preferred) or Arts
   Include certification if graduated with Latin Honors
3. Latest NMAT Result
4. Proof of Citizenship (valid PSA Birth Certificate)
5. Proof of Residency (attested by the Barangay Local Government Unit)
6. NBI Clearance (with no derogatory record) and Certificate of Good Moral Character (Certification from previous Employer and Guidance Counselor/Adviser)
7. Attestation for Indigenous Peoples from local or national authority, if applicable.
8. Recommendation from Provincial Health Officer, if with local scholarships or sponsors

   NOTE: Eligible applicants are required to submit a Research Idea Capsule after passing the CSU-SOM Admission Test.

Submit Application Form (2 sets) with complete documentary requirements enclosed in a brown expandable envelope and doubled with a transparent envelope to the School of Medicine, Batok Hall, CSU, Ampayon, Butuan City on or before May 30, 2025, 5:00 PM.
